CPC+ Offices Reminders   Christine Asato, Lead Practice Facilitator for CPC+ Hawaii Region, should have reached out to you about your Patient Roster (due last week October 6) and Care Delivery Reporting (due October 13). If you have any questions, please reach out to her ASAP at casato@mpqh.org or 808-545-2550 x6006. She also said that if you need an extension to be sure to email CPCplus@telligen.com as there is a possibility that exceptions/extensions may be granted to Hawaii practices.

HMSA Sharecare Information   Attached is information for Payment Transformation Providers regarding HMSA Sharecare Real Age Test performance bonuses. The measure will be display only for 2017 – numerator will show in COZEVA starting October 16. Special bonuses will be given to the top PCPs and IPAs in the state with the highest completion rates.  See attached letters HERE and HERE.
