Payment Transformation End-of-Year Reminders   Tiffany Pa, HMSA Provider Engagement Lead, tiffany_pa@hmsa.com (808-630-5020), should have contacted your office to review engagement measures and quality measures. Each provider must complete their PCP Engagement Measures (attestation in Cozeva and Sharecare profile) and complete the survey for the PO Performance Measure BEFORE DECEMBER 31, 2017.

  • PCP Engagement Measures

    1. Ecosystem: Attached is an updated list of PCPs who have completed Cozeva attestation. Each PCP must submit one attestation in Cozeva by December 31 and only the provider login can upload the attestation form onto Cozeva.

    2. Sharecare: Attached is an updated list of PCPs who have completed the Sharecare measure. Each PCP must have a Sharecare profile photo. Because of technical difficulties Sharecare has encountered with the logins, HMSA is considering revising the requirement to only the submittal of a photo to HMSA by December 31 to count as completed.

  • PO Performance Measures: WE NEED  YOUR HELP!!!!  No EHI PCP has completed the annual provider survey (which affects IPA revenue).  Please help us by completing before December 31 (link attached). For the question on “Medical Director,” go ahead and add Dr. Lynda Dolan, our Transformation Director, as the answer.

IPA Bonus Bucks for Payment Transformation Providers   Checks will be sent out on Friday, December 22, 2017.

CPC+ Financial Reconciliation   January 19, 2018 is the deadline for submitting 2017 financials and 2018 projections.  Deadline for getting an extension is January 5, 2018.   Attached are handouts from today’s CPC+ meeting with Christine Asato, Mountain-Pacific Quality Health, for financial reconciliation (attached), using care management fees (attached), and use of CPCP funds (attached). KTA Diabetes Self-Management Program Notice   From Dayna Wong-Otis at KTA Pharmacy: The New Year is approaching fast and now is a great time to send your referrals to KTA Pharmacy’s Diabetes Self-Management Education Program. Our next round of classes will begin on January 8, 2018 and we are looking forward to partnering with you and your patients to help them manage their diabetes. A referral (attached) to our program will help with the end-of-year push to meet the Ecosystem referral measures.
