Payment Transformation Global Payment Rates Unilateral Amendments – UPDATE HMSA will be delivering a Payment Transformation Agreement Unilateral Amendment to your offices tomorrow. This amends current rate agreements which expire on 6/30/18. HMSA is delivering this to you to meet their 60-day notice requirement. EHI received hard copies of these amendments last Friday. We are having our attorney review the amendments and did not feel comfortable distributing to members until we understood the impact. HMSA is going to distribute them anyway. Since this is a unilateral amendment, it will go into effect regardless of whether you sign it or not. EHI is planning meetings with HMSA to negotiate new rates effective 10/1/18. We want enough time to understand the proposals and negotiate and ask for more funding. The IPA will be holding meetings to review and discuss. Updated 2018 HMSA Payment Transformation GuideAttached is the new guide for HMSA Payment Transformation providers. Some changes are as follows:
An explanation of the value-based PMPM methodology, including the risk modifier PMPM and quality modifier PMPM, and a detailed example of the calculations – pages 11 to 17.
Appendix C, which explains the standardized base PMPM methodology – pages 54 to 56.
A summary of major changes is shown on page 5.

Episodic Care Management Services for Payment Transformation Providers The EHI Care Management Team is offering services for adult patients admitted to Hilo Medical Center and attributed to HMSA Payment Transformation PCPs. We ask that providers who are interested in this service sign the attached “compact” agreement outlining services and responsibilities. Please contact Susan Mochizuki at smochizuki@ehiipa.com if you are interested.
SBIRT Stands for Screening, Brief Intervention & Referral to Treatment for alcohol & substance abuse. All offices are invited to participate. Offices who collect surveys from their patients will receive a $2500 honorarium. Providers and staff will receive an additional $50 per person to attend a 2-hour workshop (regardless of participation in the program). Workshop date is Friday, May 25. Please RSVP by Friday, May 18. We will be scheduling additional workshops in June if you cannot attend the May session. EHI received a State Department of Health grant to support this program. Advanced Healthcare Directive – IMPORTANT UPDATE: Patients need to fill out the most current form. The easiest way to tell if the AHCD is the most current form is to see if it asks for the individual’s Date of Birth on the first line; and a cell phone for the Health Care Power of Attorney. USE UPDATED FORM ATTACHED. Refer your patients to a FREE session to complete their advance healthcare directives at ADRC. Dates in the flyer attached.
