Opioid Pain Contracts Sample Templates   If your office is looking for a new opioid pain contract, attached are samples here and here. As a reminder Hawai‘i State Law states, “§329-    Opioid therapy; informed consent process.  (a)  Patients and prescribers of opioids shall execute a written agreement to engage in an informed consent process if:

     (1)  A patient requires opioid treatment for more than three months;

     (2)  A patient is prescribed benzodiazepines and opioids together; or

     (3)  A patient is prescribed a dose of opioids that exceeds ninety morphine equivalent doses.”

Payment Transformation Global Payment Rates – UPDATE   EHI is initiating negotiations with HMSA for new rates effective 10/1/18. We will be scheduling meetings in the coming weeks to share information and get your feedback. Episodic Care Management Services for Payment Transformation Providers   The EHI Care Management Team is offering services for adult patients admitted to Hilo Medical Center and attributed to HMSA Payment Transformation PCPs. We ask that providers who are interested in this service sign the attached “compact” agreement outlining services and responsibilities. Please contact Susan Mochizuki at smochizuki@ehiipa.com if you are interested.

SBIRT   Stands for Screening, Brief Intervention & Referral to Treatment for alcohol & substance abuse. All offices are invited to participate. Offices who collect surveys from their patients will receive up to a $2500 honorarium. Providers and staff will receive an additional $50 per person to attend a 2-hour workshop (regardless of participation in the program). ADDITIONAL DATE ADDED: Workshop date is Friday, May 25 or Tuesday, June 5. Mountain-Pacific Quality Health – New Tools Available to Order Online   Mountain-Pacific has new resources for patients taking blood thinners that can be ordered on their website here. Topics include caring for bruises, nosebleeds and cuts while taking blood thinners. Each tool comes in a variety of formats that can be easily shared with patients.   • Advanced Healthcare Directive – IMPORTANT UPDATE: Patients need to fill out the most current form. The easiest way to tell if the AHCD is the most current form is to see if it asks for the individual’s Date of Birth on the first line; and a cell phone for the Health Care Power of Attorney. USE UPDATED FORM ATTACHED. Refer your patients to a FREE session to complete their advance healthcare directives at ADRC. Dates in the flyer attached.
