High Risk Medication Management   At the March Super Huddle, KTA introduced their High Risk Medication Management program for elderly patients. In order to reduce preventable adverse effects, KTA will notify you by FAX and suggest a medication change if a medication prescribed for your elderly patient(s) is of high risk based on 2015 Beers Criteria. Taking action based on this recommendation will help you fulfill CPC+ quality measures. Attached is a copy of KTA’s form.

HMSA Payment Transformation – ShareCare Real Age Assessment & Login Timeline   Every provider who hasn’t logged into the ShareCare website yet will be sent an email with instructions and a temporary password on Friday, May 19. The Real Age Assessment should be completed by your patients at this site: (www.hmsa.com/sharecare) by December 31, 2017 in order to complete that Payment Transformation Engagement measure. Providers must log in to receive credit for this engagement measure.

CPC+ Offices with HMSA PMSO Services – eCQM Reporting Questionnaire   First Vitals will be assisting CPC+ practices whose EMR systems cannot report eCQM measures directly to CMS. They will explain how they will do this at the upcoming workshop on May 23 in Hilo. Please complete the attached EMR questionnaire and data use agreement before attending the workshop. Also attached is an explanation of how to inform CMS when switching EHRs. You may complete and scan and email to jvitales@ehiipa.com  or FAX  to 808-935-4472 by Monday, May 22.

Sustaining Improvement Workshop #3 – Tuesday, June 6 – Noon-1:15PM   This workshop will be for CPC+ practices and focused on “after visit” activities and follow-up items from previous workshops. We will also share current IPA initiated care management/coordination initiatives. Please RSVP.

Payment Transformation Engagement and Quality Improvement Check In – for HMSA Payment Transformation providers – Tuesday, June 13Noon-1:15PM   We will share Payment Transformation Engagement and Quality Improvement year-to-date information for the IPA as a whole and for each office to see how you can best leverage your efforts to maximize your payment transformation engagement and quality improvement dollars.
