Requirements for Membership

Please have the following available when sending your membership application.

      • HMSA provider number + date credentialed
      • Curriculum Vitae
      • Short paragraph of why you would like to join the IPA

  • Licenses accepted: MD, DO, ND, APRN, PA

  • Yearly membership fee: $250.00 (plus tax)
