SBIRT-Screening, Brief Intervention, Referral to TreatmentEHI received another round of funding from the State Alcohol and Drug Abuse Division (ADAD). We are developing an iPAD version of the survey to streamline reporting and save paper! In the meantime, practices have the opportunity to receive an honorarium of $5 per correctly completed screen. Quality Measures– A number of offices have asked for help and we are prioritizing on a first-come, first-served basis. Please contact Joyce if you would like to sign up at jvitales@ehiipa.com.
Breast Cancer Screening Measure – EHI IPA staff will look at COREO to see which patients HAVE NOT had their screenings this year. We will then ask Hawaii Radiologic Associates (HRA) to double check (due to delays in COREO reporting). If your patient had a mammogram but it did not show up in COREO, we can help you input data for credit. If the patient has not scheduled a mammogram yet, HRA will call them directly on your behalf for screening appointment. PLEASE LET US KNOW IF YOU DO NOT WANT HRA TO CONTACT YOUR PATIENTS. Please contact Mary at mvitales@ehiipa.com if you want to “opt out” of this process. Otherwise we will assume that it is fine to proceed.
Annual Provider Survey – Please complete and submit this survey online before December 31, 2018.Access the link here. The IPA’s funding depends upon your completion of this survey.
Ecosystem Referrals – This measure for Payment Transformation providers will be available to submit attestation on Coreo starting Monday, November 5. Requirement for compliance is confirmation & description of referrals to programs in the community submitted via an attestation form in Coreo by the end of the measurement year (12/31/18). Each PCP needs to identify which program patients were referred to, then estimate the number of members they referred to those programs.

KTA Diabetes Prevention Program (DPP)KTA Pharmacy will begin offering the Diabetes Prevention Program beginning January 10, 2019. They are looking for patients with pre-diabetes who are willing to attend their year-long program. There is no referral required; all they need is their most recent A1c or fasting blood glucose test that shows that they have pre-diabetes. During the program, patients will learn how to make necessary lifestyle changes to lower their risk of developing type 2 diabetes. Contact KTA Pharmacy at 959-2888 to enroll your patients!
