HMSA Measure – Accepting NEW Patients + Demographics   EHI IPA providers must complete and submit the attached form by: Tuesday, February 5 (updated quarterly). This form asks if your practice is accepting NEW HMSA patients for EACH line of business along with updated office demographics. We would also like to know if you are accepting new patients for carriers other than HMSA. You can fax to 808-935-4472 or scan and email at jvitales@ehiipa.com.
